Key Facts: American Senior Communities
- Chain type: An Indiana-based operator and manager of for-profit skilled nursing facilities; among the largest in Indiana.
- Size: Tied as the thirteenth-largest nursing home chain in the United States by number of facilities — about 90 affiliated nursing homes, all in Indiana, ranked #13 of the 611 chains CMS tracks.
- Ownership: A privately held Indiana company; a number of facilities are owned by public or nonprofit entities and operated or managed by ASC under contract.
- CMS ratings: Chain-average overall rating about 3.9 of 5 (national average 3.0); about 2 of 90 facilities (2.2%) rated one star; 5 facilities (5.6%) carry a CMS abuse icon.
- Legal note: Former ASC executives were criminally convicted in a roughly $19 million fraud and kickback scheme (2016–2018); the prosecution targeted those individuals and the company itself was not charged.
- Our firm: Michael Hill Trial Law has handled more than 1,000 nursing home abuse and neglect cases nationwide, recovering hundreds of millions of dollars for clients.
Figures are from federal CMS data as cited below. CMS star ratings are relative, point-in-time measures and do not guarantee the care any individual resident receives.
Who Is American Senior Communities?
American Senior Communities (ASC) is an Indiana-based operator and manager of skilled nursing and senior-living facilities and is among the largest nursing home operators in Indiana. All of its CMS-affiliated skilled nursing facilities are in Indiana. Some facilities are operated under management arrangements, including with public hospital authorities such as the Health and Hospital Corporation of Marion County.[6]
By number of CMS-affiliated skilled nursing facilities, ASC is tied as the thirteenth-largest nursing home chain in the United States.[3]
What Kind of Chain Is American Senior Communities?
ASC is a skilled nursing facility (SNF) chain operating in Indiana. Skilled nursing facilities care for patients who need 24-hour nursing care, often after a hospital stay, and are rated on the federal CMS five-star scale.[1]
Who Owns American Senior Communities?
Within the CMS data, the ASC-affiliated facilities are operated as for-profit entities grouped under chain ID 33.[1] ASC is a privately held Indiana company. A number of its facilities are owned by public or nonprofit entities and operated or managed by ASC under contract; families should confirm the specific owning, operating, and management entities for any individual facility.[1]
How Big Is the American Senior Communities Nursing Home Chain?
American Senior Communities by the Numbers
Federal CMS data describes the operation's scale:
Facility, state, and bed counts are from the CMS Nursing Home Chain Performance Measures dataset (most recent 2026 release).[3]
American Senior Communities CMS Ratings and Staffing Data
The figures below are reported by CMS for the chain in the most recent CMS Nursing Home Chain Performance Measures release. CMS five-star ratings are relative, point-in-time measures calculated against other facilities nationally; they do not guarantee the quality of care any individual resident receives, and serious harm can occur at a facility regardless of its rating.
- The chain-average overall star rating was about 3.9 of 5 (the national chain average is 3.0). The chain-level component averages CMS publishes were 3.4 for health inspections, 2.3 for staffing, and 4.7 on the quality measure rating, which is largely self-reported.[3]
- CMS data reports 2 of 90 facilities (2.2%) with a one-star ("much below average") overall rating (national rate 20.5%) and 5 facilities (5.6%) carrying a CMS abuse icon (national rate 9.7%).[3]
- Average total nurse staffing was about 3.5 hours per resident per day (national 3.9), and reported nursing-staff turnover was about 44.0% (national 45.8%).[3]
- CMS records show 11 fines totaling $235,387 and 2 Medicare payment denials across the chain in the most recent three-year window.[3][2]
These chain-level averages describe the group as a whole and say nothing about any specific facility or any specific resident's experience. Conditions and ratings vary from home to home and change over time.
American Senior Communities Nursing Home Facilities
The table below is the complete roster of the American Senior Communities-affiliated nursing facilities (CMS chain ID 33) in the federal CMS Provider Information dataset (May 2026 release), which listed 91 facilities. It lists each facility's name, city and state, street address, and phone number, along with any current CMS quality flag. The chain-level totals cited above reflect the most recent CMS Nursing Home Chain Performance Measures release; this per-facility roster is the May 2026 provider-level snapshot and will be refreshed when CMS posts the corresponding provider-level file.[1]
The "CMS Flag" column marks homes that CMS has identified as a Special Focus Facility, a Special Focus Facility Candidate, behind on inspections ("Delayed inspection"), or carrying an abuse icon. Rows shaded gold indicate a current Special Focus designation or candidacy.[1]

Special Focus Facilities and SFF Candidates in American Senior Communities
The Special Focus Facility (SFF) program is run by CMS to identify nursing homes with a history of serious quality problems; CMS describes these facilities as among the poorest-performing nursing homes in the country. A "Special Focus Facility Candidate" is a home with a history of serious quality issues that has not yet been formally placed in the program.[4]
According to the most recent CMS Nursing Home Chain Performance Measures release, American Senior Communities had 0 Special Focus Facilities and 0 Special Focus Facility Candidates at the chain level.[3] The CMS Provider Information dataset (May 2026 release) flagged 1 facility with a Special Focus designation or candidacy and 2 facilities carrying an abuse icon at that point in time; these designations are reassessed by CMS monthly and can change.[1]

American Senior Communities Fines, Lawsuits, and Regulatory Record
The items below are drawn from federal CMS enforcement data and from the U.S. Department of Justice.
Federal Civil Money Penalties and Payment Denials
CMS enforcement records show that, across the chain, CMS imposed 11 fines totaling about $235,387 and 2 Medicare payment denials in the most recent three-year window reflected in the data.[2][3]
Criminal Prosecution of Former Executives (2016–2018)
Between 2016 and 2018, several former American Senior Communities executives were criminally prosecuted by the U.S. Department of Justice in connection with a fraud and kickback scheme. The company's former chief executive officer, James Burkhart, pleaded guilty to federal fraud, anti-kickback, and money-laundering conspiracy charges and, in 2018, was sentenced to nine and a half years in federal prison. The former chief operating officer, Daniel Benson, and other individuals were also charged. According to the Department of Justice, the scheme moved roughly $19 million through fraudulent invoices, shell companies, and kickbacks over about six years, and the funds came largely from the Health and Hospital Corporation of Marion County and from federal health programs.[7]
The prosecution was directed at these individuals, not at American Senior Communities as a company; the company itself was not charged. In December 2024, Burkhart's prison sentence was commuted as part of a broad grant of presidential clemency.[8] We note this public record for completeness; it concerns the conduct of former executives and does not, by itself, establish anything about the care provided at any individual ASC facility.
How Michael Hill Trial Law Investigates Claims Against American Senior Communities
Michael Hill Trial Law focuses exclusively on nursing home abuse and neglect. The firm has handled more than 1,000 cases against nursing home operators and has recovered hundreds of millions of dollars for clients, including a $26 million verdict in Ohio.[5]
Thin staffing and high staff turnover are among the conditions that can contribute to preventable harm in a nursing home, because residents depend on consistent, adequate staffing for basic care. A facility's overall CMS rating does not determine whether a particular resident was harmed, and serious injuries can occur at facilities with average or above-average ratings. When we investigate a claim against a American Senior Communities facility, we identify the specific entities that owned, operated, and managed the home, compare the home's CMS staffing and inspection history against what happened to the resident, and obtain the facility's care records and any available documentation. The kinds of injuries and claims our firm investigates include:
- Bedsores and pressure injuries
- Falls and fractures
- Malnutrition and dehydration
- Infections and sepsis
- Medication errors
- Elopement (wandering away from a facility)
- Understaffing and neglect
- Physical or sexual assault
- Wrongful death
We are investigating claims against facilities operated under the American Senior Communities name. If your loved one was injured or died in a American Senior Communities nursing home, the public record on this page may be relevant to your situation. Our firm serves families nationwide.
