Who Is The Ensign Group?

The Ensign Group, Inc. is a for-profit, publicly traded post-acute healthcare company. It is the largest operator of skilled nursing facilities in the United States, with more CMS-certified nursing facilities than any other company.[5]

The company was founded in 1999 in Mission Viejo, California, by Roy Christensen, his son Christopher Christensen, and Gregory Stapley. Roy Christensen had roughly fifty years in the long-term care industry. Christopher Christensen became chief executive officer in 2006 and now serves as executive chairman; Barry Port is the current chief executive officer.[6][7] The Ensign Group is headquartered in San Juan Capistrano, California.[4]

What Kind of Chain Is The Ensign Group?

The Ensign Group is primarily a skilled nursing facility (SNF) chain. Skilled nursing facilities care for patients who need 24-hour nursing care, often after a hospital stay. The large majority of Ensign's facilities are Medicare- and Medicaid-certified skilled nursing facilities, which account for the bulk of the company's revenue.[5] Ensign also operates senior living and assisted living communities and provides rehabilitation therapy and other services through affiliated entities.[4] The company calls itself a "post-acute healthcare" platform, which is an industry term for care delivered after a hospital stay.

How Big Is The Ensign Group Nursing Home Chain?

The Ensign Group by the Numbers

Federal data and the company's own filings describe a very large operation:

344
CMS-affiliated nursing homes
17
States
~37,600
Certified beds (affiliated homes)
$5.06B
FY2025 revenue

Facility, state, and bed counts are from the CMS Nursing Home Chain Performance Measures dataset (most recent 2026 release); revenue is the company's reported fiscal-year 2025 result.[28][3]

Independent reporting and corporate filings have described Ensign at varying totals because the company reports additional senior-living units beyond its CMS-certified nursing homes:

  • The most recent federal CMS chain-level data affiliates 344 nursing homes across 17 states with the Ensign chain (CMS chain ID 507) — making it the largest nursing home chain in the United States by number of facilities, ranked #1 of the 611 chains CMS tracks.[28]
  • ProPublica, using CMS affiliate data, tracks the same Ensign affiliate group (affiliate a-507).[12]
  • Investigative reporting in 2026 described Ensign as operating the most CMS-certified nursing facilities in the country, with more than 38,000 skilled nursing beds.[5]
  • The company has reported a larger total of operations when senior living and other units are included, and has continued to acquire facilities throughout 2025 and 2026.[5]

The Ensign Group, Inc. itself is a holding company. It states that care is delivered by its independent operating subsidiaries rather than by the parent directly; the company describes a deliberately decentralized model in which each facility is run as its own business with local leadership.[9]

How The Ensign Group Grew So Fast

Ensign started in 1999 with a single facility and grew through aggressive acquisitions, typically buying underperforming or distressed nursing homes and folding them into locally managed subsidiaries.[5] Key milestones include:

  • 2007: Ensign completed its initial public offering on the Nasdaq (ticker ENSG).[8]
  • 2014: Ensign spun off its real estate holdings into a separate publicly traded REIT, CareTrust REIT, Inc. (Nasdaq: CTRE), which leases many properties back to Ensign operators.[10]
  • 2019: Ensign spun off its home health, hospice, and senior living operations into The Pennant Group, Inc. (Nasdaq: PNTG).[11]
  • 2025–2026: Ensign continued rapid acquisitions, announcing in the second quarter of 2026 alone the addition of operations including 17 facilities in Texas, two in Wisconsin, and one each in Iowa and California.[5]

Ensign has stated that it targets facilities serving higher-acuity (sicker) residents, for whom government reimbursement is higher, and that newly acquired operations typically begin contributing to earnings within 12 to 18 months.[5]

The Ensign Group's Senior Living and Assisted Living Properties

Beyond its skilled nursing facilities, The Ensign Group operates a substantial and growing senior living portfolio. Ensign uses "senior living" as an umbrella term that includes independent living, assisted living, and memory care — residential settings that provide accommodations, meals, activities, housekeeping, and help with the activities of daily living for seniors who need some support but not the round-the-clock nursing care of a skilled nursing facility.[24]

This distinction matters for families. Assisted living and memory care are regulated very differently from skilled nursing facilities. They are licensed at the state level rather than under the federal Nursing Home Reform Act, the staffing and care standards vary widely from state to state, and they are not rated on the federal CMS five-star scale. As a result, an Ensign senior living community generally will not appear in the CMS nursing home facility roster above, even though it is part of the same corporate portfolio.

How Many Senior Living Properties Does Ensign Have?

Ensign reports its senior living footprint in its securities filings and acquisition announcements. The most recent figures the company has published include:

  • 48 senior living operations. As of June 1, 2026, Ensign reported that its growing portfolio had reached 396 healthcare operations, including 48 senior living operations, across 17 states.[25]
  • 3,402 senior living units. In its annual report for the year ended December 31, 2025, Ensign reported an aggregate of 3,402 senior living units across 47 operations, of which 31 were located on the same site as one of its skilled nursing operations (a "campus" setting).[26]
  • A dozen states. Ensign's senior living communities are located in Alaska, Arizona, California, Colorado, Idaho, Iowa, Kansas, Nebraska, Oregon, Texas, Utah, and Washington.[26]
  • A small share of revenue. Ensign reports that revenue from its senior living operations, together with mobile diagnostics, transportation, other real estate, and other ancillary operations, makes up roughly 4.6% of its annual revenue — the company's business remains dominated by skilled nursing.[26]

The senior living count has climbed steadily as Ensign has acquired new communities. The company reported 44 senior living operations in April 2025 and 48 by June 2026, and its 2025 and 2026 acquisition announcements regularly bundle assisted living, memory care, and independent living units alongside skilled nursing beds.[27] Recent examples include the June 2026 purchase of the real estate to Memory Care of Contra Costa, a 46-unit memory care facility in Pleasant Hill, California, and a May 2026 Texas and Wisconsin expansion that added assisted living and independent living units — for instance, a campus in Odessa, Texas with 30 assisted living units and 55 independent living units.[25][27]

The Role of Standard Bearer and Third-Party Operators

Many of Ensign's senior living properties are held by Standard Bearer Healthcare REIT, Inc., Ensign's captive real estate company, which owns properties and leases them out under long-term triple-net leases.[25] As of December 31, 2025, Standard Bearer's portfolio comprised 152 real estate properties; 116 were leased to Ensign's own independent subsidiaries and 37 were leased to operations owned and managed by third-party operators.[26] By June 2026, Ensign subsidiaries (including Standard Bearer) reported owning 181 real estate assets.[25]

This means a given Ensign-affiliated senior living community may be owned by an Ensign entity but operated day-to-day by an unrelated company under a lease — another reason the corporate structure behind any individual property is worth confirming when a resident is harmed. If your loved one was injured in an assisted living, memory care, or independent living community connected to Ensign, our firm can help identify which entities owned, leased, and operated the community.

Is The Ensign Group For-Profit or Nonprofit? Revenue and Net Worth

The Ensign Group is a for-profit company. It is publicly traded on the Nasdaq under the ticker symbol "ENSG."[4] Within the CMS data, the Ensign-affiliated facilities are operated almost entirely as for-profit entities.[1]

The Ensign Group's reported finances include:

  • 2025 revenue: about $5.06 billion, an increase of roughly 18.7% over the prior year.[3]
  • 2025 net income: about $344.0 million (GAAP), up roughly 15.4% year over year.[3]
  • 2026 guidance: annual revenue guidance of roughly $5.77 billion to $5.84 billion.[3][13]
  • Market value (net worth): reported in the range of roughly $10 billion in recent years.[5]

Like other large nursing home operators, Ensign depends heavily on government healthcare programs; the bulk of its revenue is derived from Medicare and Medicaid.[5] The company reports its business in two segments: Skilled Services (the operation of nursing facilities and rehabilitation therapy) and Standard Bearer (a real estate arm that owns properties and leases them to operators).[4]

The Ensign Group Nursing Home Facilities

The table below is the complete roster of the 338 nursing facilities affiliated with The Ensign Group (CMS chain ID 507), taken directly from the federal CMS Provider Information dataset (May 2026 release). It lists each facility's name, city and state, street address, and phone number, along with any current CMS quality flag. (The chain-level totals cited above reflect the newer CMS Nursing Home Chain Performance Measures release, which affiliates 344 facilities with the chain; this per-facility roster is the May 2026 provider-level snapshot and will be refreshed when CMS posts the corresponding provider-level file.) Ensign's facility list changes as the company acquires and divests properties, so this roster should be refreshed from the CMS data on each republish.[23]

The "CMS Flag" column marks homes that CMS has identified as a Special Focus Facility, a Special Focus Facility Candidate, behind on inspections ("Delayed inspection"), or carrying an abuse icon. Rows shaded gold indicate a current Special Focus designation.[23]

Source: Centers for Medicare & Medicaid Services, Provider Data Catalog, Nursing Home "Provider Information" dataset (NH_ProviderInfo_May2026.csv), filtered to The Ensign Group, chain ID 507. Inclusion on this list reflects CMS chain-affiliation data and does not by itself indicate that any particular facility committed wrongdoing.[23]

Special Focus Facilities and SFF Candidates in The Ensign Group

The Special Focus Facility (SFF) program is run by CMS. It identifies nursing homes with a history of serious quality problems. CMS describes these facilities as among the poorest-performing nursing homes in the country.[14] A "Special Focus Facility Candidate" is a home with a history of serious quality issues that has not yet been formally placed in the program.[12]

According to the federal CMS Provider Information dataset (May 2026 release), the Ensign Group chain included 1 Special Focus Facility, 7 Special Focus Facility Candidates, and 19 homes whose most recent standard health inspection was more than two years ago. CMS data also showed 30 Ensign facilities carrying an abuse icon, meaning they were cited for abuse or neglect at or above a defined harm level.[23]

These designations are reassessed by CMS monthly and can change. The most recent CMS Nursing Home Chain Performance Measures release reports 0 Special Focus Facilities and 6 SFF candidates for the Ensign chain; the specific facilities named below reflect the CMS May 2026 provider-level release.[28]

The Ensign Group Quality and Staffing Record

Federal data offers a mixed picture of the chain. Across the Ensign-affiliated chain in the most recent CMS Nursing Home Chain Performance Measures release (344 facilities in 17 states):

  • The average overall star rating was about 3.2 of 5. The chain-level averages CMS publishes were 2.9 for health inspections and 2.7 for staffing — both below the midpoint — alongside a higher 4.4 average on the quality measure rating, which is largely self-reported.[28]
  • Within the chain, 47 facilities (about 13.7%) carried a one-star ("much below average") overall rating — below the roughly 20.5% national rate — while a larger share carried four or five stars.[28]
  • The affiliated facilities collectively recorded 283 fines totaling about $6.84 million and 23 Medicare payment denials in the most recent three-year window reflected in the data. Average total nurse staffing was about 3.7 hours per resident per day (below the national average of roughly 3.9), and reported nursing-staff turnover was about 44.9%.[28][2]
  • Of the 333 Ensign facilities appearing in the FY2026 federal SNF Value-Based Purchasing dataset, 281 received a downward Medicare payment adjustment for the program year.[15]

A 2026 investigation by Hunterbrook Media — which disclosed that an affiliated fund held a short position in Ensign stock — alleged, based on its analysis of CMS data and company filings, that Ensign tends to perform better on self-reported quality metrics than on independently verified ones, and that its facilities fell below state minimum-staffing requirements in California, Washington, Tennessee, and Kansas on more than 18,000 days cumulatively between 2020 and 2025.[5] These are the conclusions and opinions of the reporting organization and the experts it quoted; they have not been adjudicated in court. Ensign markets itself on its clinical outcomes and states that it has a history of improving the facilities it acquires.[5]

The Ensign Group Lawsuits, Federal Settlements, and Sanctions

Ensign entities have twice resolved major federal fraud matters with payments exceeding $40 million. In each case, the settlement resolved allegations without an admission of liability. The events below are drawn from government records, court filings, news reporting, and the company's own securities filings.

2013 — $48 Million False Claims Act Settlement (Unnecessary Therapy)

In November 2013, The Ensign Group agreed to pay $48 million to resolve U.S. Department of Justice allegations that six of its California skilled nursing facilities knowingly submitted false Medicare claims for medically unnecessary rehabilitation therapy.[16][17] The six facilities named were Atlantic Memorial Healthcare Center (Long Beach), Panorama Gardens (Panorama City), The Orchard Post-Acute Care / Royal Court (Whittier), Sea Cliff Healthcare Center (Huntington Beach), Southland (Norwalk), and Victoria Care Center (Ventura).[16] The case arose from whistleblower (qui tam) lawsuits filed under the False Claims Act. A U.S. Attorney described it at the time as one of the largest settlements of its kind.[17]

2025 — $47.3 Million Settlement (Alleged Kickbacks for Referrals)

In a matter announced in September 2025, The Ensign Group, Inc. and Ensign Services, Inc. agreed to pay more than $47.3 million to resolve a whistleblower lawsuit originally filed in 2015 under the federal and California False Claims Acts.[18] The suit alleged that Ensign knowingly paid illegal kickbacks to physicians — described as inflated consulting and medical-director fees — in exchange for patient referrals to its skilled nursing facilities, in alleged violation of the Anti-Kickback Statute and the Stark self-referral law.[18][19] Ensign's own securities filings disclosed that the parties agreed in January 2024 to settle the civil case, subject to government review.[20] The settlement resolved the allegations without an admission of liability.

Congressional Scrutiny of Industry Finances

In May 2024, U.S. Senator Elizabeth Warren and several colleagues sent a letter directly to Ensign's then-executive chairman questioning the gap between large executive pay, dividends, and stock buybacks on one hand and the nursing home industry's claims that it could not afford higher staffing on the other. The letter characterized certain related-party payment practices common in the industry as a "deceptive tactic" used to obscure profits.[21] The letter reflects the senators' assertions; it is not a finding of wrongdoing.

Medicare and Medicaid Allegations Against The Ensign Group

Ensign's two largest federal matters both centered on how the company billed Medicare and Medicaid. The allegations included:

  • Billing Medicare for medically unnecessary rehabilitation therapy at six California facilities (resolved by the 2013 settlement).[16]
  • Paying illegal kickbacks to physicians, disguised as consulting and medical-director fees, in exchange for patient referrals (resolved by the 2025 settlement).[18]

It is important to be precise about what has and has not been established. Both federal matters described above were resolved through civil settlements in which Ensign did not admit liability. The independent reporting and congressional correspondence referenced on this page reflect allegations, analyses, and opinions of their authors. As of the date this page was prepared, no court has found The Ensign Group liable for Medicare or Medicaid fraud.

How Michael Hill Trial Law Investigates Claims Against The Ensign Group

Michael Hill Trial Law focuses exclusively on nursing home abuse and neglect. The firm has handled more than 1,000 cases against nursing home operators and has recovered hundreds of millions of dollars for clients, including a $26 million verdict in Ohio.[22]

Our firm has been involved in multiple cases involving Ensign Group facilities and has a record of seven-figure settlements in nursing home abuse and neglect matters.[22]

Financial pressure on a large, fast-growing chain can affect care at the bedside. When staffing is thin, when administrators are stretched across multiple buildings, or when a facility carries a history of serious deficiencies, residents can suffer preventable harm. When we investigate a claim against an Ensign Group facility, we compare the home's CMS staffing and inspection history against what happened to the resident, obtain the facility's care records and any available documentation, and work to identify which Ensign operating subsidiary, landlord, and management entity were responsible for the resident's care.
