Key Facts: Saber Healthcare Group

  • Chain type: Privately held, for-profit operator of skilled nursing and rehabilitation facilities.
  • Size: The seventh-largest nursing home chain in the United States by number of facilities — about 126 affiliated nursing homes across 6 states (Delaware, Indiana, North Carolina, Ohio, Pennsylvania, and Virginia), ranked #7 of the 611 chains CMS tracks.
  • Founded / headquarters: Founded in 2001; headquartered in Bedford Heights, Ohio.
  • Ownership: Privately held, for-profit; does not publicly report revenue.
  • CMS ratings: Chain-average overall rating about 2.9 of 5 (national average 3.0); CMS data reports 28 of 126 facilities (22.2%) at one star and 12 (9.5%) with an abuse icon. Ratings are point-in-time measures and do not guarantee any individual resident’s care.
  • Federal settlement: In 2020, Saber entities agreed to pay $10 million to resolve False Claims Act allegations of billing for unnecessary therapy — resolved as allegations only, with no determination of liability — and entered a five-year Corporate Integrity Agreement.
  • Our firm: Michael Hill Trial Law has handled more than 1,000 nursing home abuse and neglect cases nationwide, recovering hundreds of millions of dollars for clients.

Figures are from federal CMS data as cited below; the litigation described is characterized accurately, including its outcome. CMS star ratings are relative, point-in-time measures and do not guarantee the care any individual resident receives.

Who Is Saber Healthcare Group?

Saber Healthcare Group is a for-profit, privately held company that owns and operates skilled nursing and rehabilitation facilities. By number of CMS-affiliated skilled nursing facilities, it is the seventh-largest nursing home chain in the United States.[3]

The company was founded in 2001 and is headquartered in Bedford Heights, Ohio, in the Cleveland area.[4][5] It is privately held and does not publicly report its financial results.[4]

What Kind of Chain Is Saber Healthcare Group?

Saber is primarily a skilled nursing facility (SNF) chain, and it also provides rehabilitation and related services at its facilities. Skilled nursing facilities care for patients who need 24-hour nursing care, often after a hospital stay, and are rated on the federal CMS five-star scale. Saber operates its facilities through a network of affiliated operating entities, so the corporate and management structure behind any individual facility is worth confirming when a resident is harmed.[4]

How Big Is the Saber Healthcare Group Nursing Home Chain?

Saber by the Numbers

Federal CMS data describes the following:

126
CMS-affiliated nursing homes
6
States (DE, IN, NC, OH, PA, VA)
13,568
Certified beds
#7
Largest U.S. chain (of 611)

Facility, state, and bed counts are from the CMS Nursing Home Chain Performance Measures dataset (most recent 2026 release); the company is privately held and does not publicly report revenue.[3]

  • The most recent federal CMS chain-level data affiliates 126 nursing homes across Delaware, Indiana, North Carolina, Ohio, Pennsylvania, and Virginia with the chain (CMS chain ID 461) — ranking it #7 of the 611 chains CMS tracks by number of facilities.[3]
  • The affiliated homes hold about 13,568 certified beds.[3]

Is Saber Healthcare Group For-Profit or Nonprofit?

Saber is a for-profit company. It is privately held rather than publicly traded, and it does not publicly report revenue or profit.[4] Within the CMS data, the Saber-affiliated facilities are operated as for-profit entities.[1] Like other large nursing home operators, the company depends heavily on government healthcare programs; Medicare and Medicaid reimbursement account for a substantial share of skilled nursing revenue industrywide.

Saber Healthcare Group CMS Ratings and Staffing Data

The figures below are reported by CMS for the chain in the most recent CMS Nursing Home Chain Performance Measures release (126 facilities). CMS five-star ratings are relative, point-in-time measures calculated against other facilities nationally; they do not guarantee the quality of care any individual resident receives, and serious harm can occur at a facility regardless of its rating.

  • The chain-average overall star rating was about 2.9 of 5 (the national chain average is 3.0). The chain-level component averages CMS publishes were 2.6 for health inspections, 2.2 for staffing, and 4.0 on the quality measure rating, which is largely self-reported.[3]
  • CMS data reports 28 of 126 facilities (22.2%) with a one-star ("much below average") overall rating (national rate 20.5%) and 12 facilities (9.5%) carrying a CMS abuse icon (national rate 9.7%).[3]
  • Average total nurse staffing was about 3.4 hours per resident per day (national average about 3.9), and reported nursing-staff turnover was about 47.3% (national 45.8%).[3]
  • CMS records show 115 fines totaling about $3.08 million and 8 Medicare payment denials across the chain in the most recent three-year window.[3][2]

Saber Healthcare Group Nursing Home Facilities

The table below is the complete roster of the 126 nursing facilities affiliated with Saber Healthcare Group (CMS chain ID 461), taken directly from the federal CMS Provider Information dataset (May 2026 release). It lists each facility's name, city and state, street address, and phone number, along with any current CMS quality flag. The chain-level totals cited above reflect the most recent CMS Nursing Home Chain Performance Measures release, which affiliates the same 126 facilities with the chain. The facility list changes as the company acquires and divests properties, so this roster should be refreshed from the CMS data on each republish.[1]

The "CMS Flag" column marks homes that CMS has identified as a Special Focus Facility, a Special Focus Facility Candidate, behind on inspections ("Delayed inspection"), or carrying an abuse icon. Rows shaded gold indicate a current Special Focus designation or candidacy.[1]

Special Focus Facilities and SFF Candidates in Saber Healthcare Group

The Special Focus Facility (SFF) program is run by CMS to identify nursing homes with a history of serious quality problems; CMS describes these facilities as among the poorest-performing nursing homes in the country. A "Special Focus Facility Candidate" is a home with a history of serious quality issues that has not yet been formally placed in the program.[7]

According to the federal CMS Provider Information dataset (May 2026 release), the Saber chain included 1 Special Focus Facility, 3 Special Focus Facility Candidates, and 18 homes whose most recent standard health inspection was more than two years ago. CMS data also showed 12 facilities carrying an abuse icon, meaning they were cited for abuse or neglect at or above a defined harm level.[1]

These designations are reassessed by CMS monthly and can change. The most recent CMS Nursing Home Chain Performance Measures release reports 0 Special Focus Facilities and 3 SFF candidates for the chain; the specific facilities named below reflect the CMS May 2026 provider-level release.[3]

Saber Healthcare Group Lawsuits, Federal Settlements, and Regulatory Record

The items below are drawn from federal records, court filings, and CMS enforcement data. Where a settlement is described, its outcome is stated.

2020 — $10 Million False Claims Act Settlement (Unnecessary Therapy)

In April 2020, Saber Healthcare Holdings, LLC and related entities agreed to pay $10 million to resolve U.S. Department of Justice allegations that they violated the False Claims Act by knowingly causing certain skilled nursing facilities to submit false Medicare claims for rehabilitation therapy that was not reasonable, necessary, or skilled.[5] The government alleged that Saber set company-wide expectations that patients receive the highest ("Ultra High") level of therapy regardless of individual clinical need. The matter arose from a whistleblower (qui tam) lawsuit filed by former Saber therapists and therapy managers.[5] As part of the resolution, Saber entered a five-year Corporate Integrity Agreement with the U.S. Department of Health and Human Services, Office of Inspector General, requiring independent annual review of the medical necessity of its Medicare therapy billing.[6] The settlement resolved allegations only; there was no determination of liability.

Federal Civil Money Penalties and Payment Denials

Separately, CMS enforcement records show that, across the chain, CMS imposed 115 fines totaling about $3.08 million and 8 Medicare payment denials in the most recent three-year window reflected in the data. These are regulatory penalties recorded by CMS and reflect the chain's inspection and compliance history.[2][3]

It is important to be precise about what has and has not been established. The 2020 federal matter was resolved through a civil settlement in which Saber did not admit liability. The CMS ratings, fines, and Special Focus data on this page are federal regulatory records; they describe inspection findings and penalties, not court judgments of fraud.

How Michael Hill Trial Law Investigates Claims Against Saber Healthcare Group

Michael Hill Trial Law focuses exclusively on nursing home abuse and neglect. The firm has handled more than 1,000 cases against nursing home operators and has recovered hundreds of millions of dollars for clients, including a $26 million verdict in Ohio.[8]

Thin staffing and high staff turnover are among the conditions that can contribute to preventable harm in a nursing home, because residents depend on consistent, adequate staffing for basic care. A facility's overall CMS rating does not determine whether a particular resident was harmed. When we investigate a claim against a Saber facility, we compare the home's CMS staffing and inspection history against what happened to the resident, obtain the facility's care records and any available documentation, and work to identify which operating subsidiary, landlord, and management entity were responsible for the resident's care. The kinds of injuries and claims our firm investigates include:

We are investigating claims against facilities operated by Saber Healthcare Group. If your loved one was injured or died in a Saber nursing home, the public record on this page may be relevant to your situation. Our firm serves families nationwide.

Ratings, fines, and Special Focus data are from federal CMS records and reflect inspection findings and regulatory penalties, not court judgments; the federal settlement described resolved allegations only, with no determination of liability. CMS star ratings are relative, point-in-time measures and do not guarantee the care any individual resident receives. Facility data and litigation status change over time; verify current details before relying on them. Last reviewed: October 26, 2026.
